Every figure that appears is a sample of how these plans commonly treat children in Idaho, not a quote on your plan. Employer groups differ enormously, even inside one carrier, which is why the questions further down matter more than any table.

The seven terms that decide what you pay
Benefit year
The twelve months your plan counts in. Most reset on 1 January, some on your employer's anniversary date. Nothing unused carries over, which is why December books out.
Coverage tiers
Plans sort treatment into preventive, basic and major, then pay a different percentage of each. A cleaning is preventive, a filling is basic, a crown is major. The tier a treatment sits in matters more than its price.
Frequency limits
How often the plan will pay for the same thing. Two cleanings a year is standard, and some plans count six months to the day rather than twice per calendar year.
Age limits
The pediatric one parents miss. Sealants and fluoride are often covered only up to a set age, commonly fourteen or sixteen, and sealants only on permanent molars. Place them while they are covered.
Waiting periods
New plans often make you wait six to twelve months before basic or major work is paid for. Preventive care is usually available from day one. Ask whether a previous employer's coverage waives it.
Orthodontic lifetime maximum
A separate pot from the rest of the plan, paid once per person rather than once per year, and often capped by age. Phase one treatment at eight can use up the money you wanted at fourteen.
Coordination of benefits
When a child is on both parents' plans, the birthday rule usually decides who pays first: the parent whose birthday falls earlier in the calendar year, regardless of who is older. Send us both cards.

What is left of the annual maximum for this child, not the family?
Are sealants covered, on which teeth, and up to what age?
How many cleanings and fluoride treatments a year, counted by calendar or by month?
Is there a waiting period on basic or major treatment, and when does it end?
What is the orthodontic lifetime maximum, and does phase one treatment draw on it?
Is this practice in network, and if not, what does the plan pay out of network?
If my child is on two plans, which is primary under the birthday rule?
